Application form for registration/entry to ICS CZ

You can only fill the application form if your horse is registered in the foreign Irish Cob Stud book and you provide a Zootechnical certificate according to EU Regulation 2016/1012.

In all other cases, the participation of the horse in the inspection is required!!! (fill the application form to inspection)

Necessary attachments: Zootechnical certificate, photocopy (scan) of the horse's passport, pedigree and 3 photos (left, right, front)

To enter the assessments (premiums) from ICS NL: a copy of the inspection sheet or the assessment page in the passport before 31/12/2020

ATTENTION! Attachments must not exceed the 15 MB limit, otherwise it is necessary to send two attachments.
